I've had one where my villager feels a sting on her leg.
The choices were to swat at it or watch.
I chose watch and it was a spider and ant fighting. Nothing happened but I feel that swatting it would have caused something bad.

I also had one with a pearl, don't remember color.
It was to crush it or something else.
I chose crush it and my villager felt energetic and became a runner.
