What we mean wehn we say we finished the game with no-one dying is that we completed 15 of the 16 puzzles, and the only puzzle left is the graveyard puzzle, which does require death.

I've finished 15/16, with my oldest villager being 49 at the time. It's just another way of challenging yourself in the game \:\)
