I have noticed/wondered this a while now... I watched my eldest like a mother hen and his younger "healthier" brother died......
Is the game programed to only let vv's die when you are not actually looking at them? Or are all we just careless??!.....

I was playing on hard and was very low on food. With three of my villagers weakened to the point that any moment was their last, I had to alternate between puttin each at the food bin to try and up their strenght. I wasn't quick enough, as I went to pick her up she died. 'Poof' Unfortunately none of that village made it. Pesky flock of birds, where's a slingshot when ya need it

I've had a villager "die in my arms" while I was dragging her.

Hi, been gone awile glad to be back.

I had a villager die while "burying the dead"
and I had a ghost once (glitch??) she just kept walking back and forth past the well but I couldn't pick her up or select her in any way.

I went away for just a little bit and came back to find one of the men got one of my elderly women pregnant with twins! I thought there was no way she'd make it. But she did. And sadly as soon as she released the babies she became a skeleton!

I had this Master Farmer named City, one of my favorites, who died at about 80 (I had level 2 meds). None of my other dearly departed had lived that long, I was sure she'd be dead when I got on...but she lived! Then I was looking at the research table--I think she was fishing or something--and suddenly there was this dead body right in the middle of the village square! Kinda freaky...
