I'm confused - the guide says at level 3 Medicine my villagers will build a hospital, but there's already a hospital there at the beginning of the game. Are there going to be two hospitals? Is it an upgrade or rebuild of the current hospital?

No there is only one Hospital. When you get level 3 it is just upgraded. Has beds ect: You dont have to build it.
