Not that I want someone to become very sick, but one of the few trophies I don't have yet on one of my families is the gravely ill one. How do I get this? Do I have to refrain from treating a sick person until they become gravely ill?

I was able to get it by

having an illness that required a shot of Vancomycin to cure. She was sick with a sore throat -- after I cured her, she was sick again. I used a Doctor consult, and he told me to use Vancomycin. I got the trophy after that -- no need to let them get very sick on the health bar.

you just have to buy a bag of food from each of the 4 food groups in 24 hours, at least it worked for me that way. Of course, I completed it while still on version 1 rather than 1.5 -- might have changed.

I don't think you can force a peep to become gravely ill. It just happens. The best way to recognise it is:

The peep keeps becoming sick with a variety of ailments, sore throat, itchy, sneezing, etc. Get a doctor's diagnosis and usually a shot of vancomycin will be prescribed.

So far my peeps are like my "real" family, they rarely get sick. I haven't had to give my peeps antibiotics even once and only had to give medicine a few times.
