you need level two of medicine technology in order to train doctors. you can click on "tech" and then click on the question marks next to the buy buttons for the types of technologies to learn more about them and what they do.
You can also gain healing skill by actually healing any villager who gets sick. Watch for the chief to go to the hospital area and when you click on him it says something like, "Concerned about the sick." Then you can find whoever is sick - although eventually they will make their way to the hospital. Sometimes they also hang around the food bin.
