Some of my Villagers show up as "weakened" but never sit/cough to allow them to be healed - if a doctor is placed over them they just run away...

Any ideas as they keep dying off?

There's a difference between "weakened" and "sick" villagers. You can only heal "sick" villagers. Villagers become weak for a variety of reasons: prolonged sickness, lack of food, on-the-job injuries. The only thing you can do to help weak villagers is to make sure they have plenty of food, have them work at a less strenuous job, and purchase higher levels of medical technology.

Farmers always seem to be healthier than any other villagers. I've heard an accusation that they're sneaking food out of the baskets on the way to the food bin!

I have tried placing him in the "calm and peaceful clearing" but he just gets confused and leaves....AAARRGH

Then you need to have your villagers do a little more research!
